• Photo ID and insurance information, if you’re using insurance.
• Comfortable clothing: If you’re scheduled for a physical treatment, wearing or bringing clothes you can easily move in can help.
• Medical records or prescriptions: If there’s any recent documentation that may be relevant, feel free to bring it along.

We have convenient parking available, and once you come inside, our front desk team will help you check in. If you need to complete any forms, they’ll provide you with everything you need. We keep a comfortable waiting area, and we’re here if you have questions.
